Terms and Conditions for Patients – Mora & Valdez Dentistry
By scheduling an appointment, receiving treatment, or using our services, you agree to the following Terms and Conditions, which ensure transparency, safety, and an optimal experience for all patients.
1. Initial Evaluation and Treatment Plan
Every treatment begins with a comprehensive clinical evaluation, which may include panoramic X-rays, CBCT (CT Scan), and/or cephalometric radiographs as needed.
The proposed treatment plan is a personalized diagnosis based on your oral health and may change if clinical conditions change.
The patient understands that dental treatment may require modifications, additional visits, or complementary procedures to achieve optimal results.
2. Informed Consent
Before starting any procedure, an informed consent form will be provided and explained.
No treatment will be performed without prior authorization from the patient or legal representative.
3. Payments, Deposits, and Financial Policy
Some treatments require an initial deposit to reserve the appointment, especially surgeries, implants, orthodontics, and Invisalign.
Payments may be made in cash, by card, or bank transfer.
Treatment fees are provided in a formal estimate. Any changes to the treatment plan may modify the final cost.
Balances must be paid the same day as the treatment unless a written agreement states otherwise.
4. Cancellation Policy
Appointments must be canceled or rescheduled at least 24 hours in advance.
Late cancellations or no-shows may result in an administrative fee or forfeiture of deposit, especially for surgical or long appointments.
Surgeries require confirmation 48 hours in advance.
5. Guarantees and Scope
Dental treatments generally have very good outcomes, but results may vary depending on:
the patient’s oral hygiene
habits (bruxism, smoking, etc.)
compliance with follow-ups
medical conditions
No absolute guarantees are offered since no medical/dental procedure can ensure exact results.
6. Images, X-Rays, and Records
All radiographic and clinical records are part of the patient’s medical history.
Patients may request digital copies at any time.
Images may be used for educational or academic purposes only with written authorization.
7. Medications and Home Care
The patient is responsible for following all preoperative and postoperative instructions.
Any adverse reactions must be reported immediately.
8. Confidentiality and Data Protection
All patient information is handled with strict confidentiality.
Mora & Valdez Dentistry complies with applicable privacy and data protection regulations.
Information will only be shared with third parties when necessary for treatment or when legally required.
9. Patient Responsibilities
The patient agrees to:
provide truthful medical and dental information,
attend appointments on time,
follow clinical recommendations,
report changes in general health or medications.
10. Limitation of Liability
The clinic is not responsible for complications arising from:
